Abstract Summary
Rare case of primary scrotal lymphoma in an 81-year-old man initially mimicked testicular cancer but proved to be aggressive diffuse large B-cell lymphoma. Unusual “skip metastasis” to mediastinum without regional lymph node involvement highlighted the disease’s atypical behavior. With >90% Ki-67 index and stage IV presentation, the patient died within 3 months after refusing treatment—underscoring the critical need for early biopsy and heightened clinical suspicion when evaluating scrotal masses.
