Smoking-attributable ischemic stroke deaths in China declined from 1990-2021, with sharper drops in women (-2.65%) than men (-0.58%). Projections suggest continued decline through 2035, but demographic aging and past smoking patterns may slow progress. Targeted tobacco control for middle-aged and older men is critical to sustain momentum and prevent future resurgence.
