Efficacy and toxicity of neoadjuvant radiotherapy with concomitant dose escalation for rectal cancer.

Abstract Summary

Intensified neoadjuvant radiotherapy (55-56 Gy) followed by surgery achieved impressive results in 415 rectal cancer patients: 23.9% complete pathological response and 86.5% sphincter preservation. Waiting >10 weeks between radiation and surgery improved response rates. Patients with complete response showed significantly better disease-free survival (83% vs 63%), with acceptable side effects.

Higher-dose neoadjuvant radiotherapy for rectal cancer achieves 23.9% complete response rate with acceptable toxicity and improved survival, especially when surgery is delayed beyond 10 weeks.
